GENERAL INFORMATION:

Insurance:

Dr. Welty is not on any panels for insurance so the office does not accept insurance co-pays.  Payment is required at each visit.  This does not mean that patients with insurance will not be able to see Dr. Welty.  At the end of the appointment, the patient will be given a Health Insurance Claim Form with the diagnosis code, paid amount, patient information, and Dr. Welty's information.  Several of our current patients send this form into their insurance provider to be reimbursed for their visits.   Please keep in mind that reimbursement is between the patient/parent and their insurance company.  Please contact them for regarding specific situation.  Our office does not handle any medical billing.

Appointments:

It is extremely important that every scheduled appointment be kept.  No shows and cancellations interfere with the pace of your progress and prevent the scheduling of others who want to be seen.  Last minute interruptions sometimes happen and cannot be foreseen.  The policy of this office is to allow a total of two episodes of appointment no shows, or appointment cancellations without 24 hours notice.  After the second no show or cancellation with less than 24 hours notice, our office will charge the entire fee for any further scheduled and missed appointments.

Qualified Medical Evaluation (QME) appointments require at least a 72 hours notice when cancelling to make room for other patients that want to be seen.   If not cancelled 72 hours in advance, a minimum fee of $450.00 will be charged for the missing appointment.  Please see Psychiatric Services for QME/AME locations and information.
